For seventy years, fluoride has been the gold standard in cavity prevention — and for good reason. But a newer ingredient, nano-hydroxyapatite (n-HAp), is showing up on more tubes and in more dental chairs. So is it actually better? The honest answer: for most adults, it's comparable. For some people, it's a smarter fit. Here's what the research shows.
What hydroxyapatite actually is
Hydroxyapatite is the mineral your enamel is already made of — roughly 97% of it. Nano-hydroxyapatite is a synthesized, ultra-fine version that can fill microscopic surface defects and integrate with existing enamel, a process clinicians call remineralization.
How it stacks up against fluoride
Multiple randomized clinical trials over the past decade — including a notable 18-month study published in Clinical Oral Investigations — have found 10% nano-hydroxyapatite non-inferior to 500 ppm fluoride for caries prevention in adults. That is a meaningful result: it does the job, without relying on fluoride's mechanism.
Who should consider switching

Who should stick with fluoride

Fluoride still has the longest and largest body of evidence behind it, and prescription strengths are not yet matched by hydroxyapatite formulas on the market.

What to look for on the label
Aim for 10% nano-hydroxyapatite as the active ingredient. Avoid SLS if you get canker sores. Skip the charcoal blends — they are abrasive and dull the benefit. A simple, mild mint or unflavored paste is usually the cleanest pick.
Both fluoride and hydroxyapatite work. Pick the one you will actually use twice a day, every day. That habit is the variable that matters most.
